Certified longevitydocs.™️ (CLD) Program Agreement

By checking the box indicating acceptance and completing enrollment in the Certified longevitydocs.™ (CLD) Program, I acknowledge that I have read, understood, and agree to be legally bound by the terms of this Agreement with Longevity Docs LLC (“Longevity Docs,” “we,” or “us”).

1. Program Overview & Eligibility

The Certified LongevityDocs.™ (CLD) Program is an advanced, physician-focused educational program designed exclusively for licensed MDs, DOs, or international equivalents. The program emphasizes scientific rigor, clinical application, and evidence-based longevity medicine.

By enrolling, I represent that I meet the eligibility requirements and understand that the program demands active participation and professional conduct.

2. Tuition & Payment Terms

  • Total Tuition: $10,000 USD per participant

  • Payment Requirement: Tuition must be paid in full or pursuant to an approved installment plan to confirm enrollment.

  • Access Timing: Program access will be granted within three (3) months of enrollment.

  • Non-Refundability: Tuition is non-refundable once the program has officially commenced, reflecting the value of the reserved seat, proprietary materials, and limited-capacity cohort participation.

Installment plans, if offered, are administered by third-party providers (e.g., Klarna). Approval, payment schedules, and terms are determined by the provider.

Failure to remain current on payments may result in suspension of program access.

3. Participation & Engagement

I agree to engage fully with the CLD curriculum, which includes over 100 hours of educational content such as video lectures, written materials, assessments, live sessions, case studies, and research reviews.

I understand that the value of the program depends on active participation, collaboration, and adherence to professional and ethical standards.

4. Peer & Faculty Engagement

I agree to:

  • Participate respectfully in live sessions, discussions, and peer interactions

  • Contribute to a collaborative and collegial learning environment

  • Share insights and experiences within appropriate professional and ethical boundaries

5. Confidentiality

I acknowledge that program participation may involve confidential or sensitive discussions. I agree to:

  • Maintain the confidentiality of cohort discussions

  • Refrain from sharing identifiable patient, peer, or proprietary information without proper authorization

  • Respect the privacy and professional integrity of all participants

6. Feedback & Program Development

As a participant—particularly within a founding cohort—I agree that any feedback, suggestions, or evaluations I provide may be used by Longevity Docs LLC to improve the program without obligation of confidentiality, attribution, or compensation. Such feedback does not grant me intellectual property rights.

7. Intellectual Property & Use of Credential

All program materials—including videos, written content, assessments, templates, protocols, and structure—are the exclusive intellectual property of Longevity Docs LLC.

I agree not to reproduce, distribute, or adapt any materials outside my personal participation without express written consent.

The Certified LongevityDocs.™ (CLD) name, logo, and branding are proprietary. Upon successful completion, I may use the credential solely to represent my personal certification and may not imply endorsement or unauthorized affiliation.

Permitted example:
“Jane Smith, MD, CLD”

8. Certification & Future Requirements

Certification is awarded upon successful completion of the program and fulfillment of all payment obligations.

Longevity Docs LLC reserves the right to introduce future updates, continuing education, or recertification requirements to maintain alignment with clinical and scientific advancements.

9. Professional Conduct & Enforcement

I agree to uphold the highest standards of professionalism, integrity, and collegiality. Failure to comply with this Agreement may result in review, suspension, or revocation of certification at the discretion of Longevity Docs LLC.

10. Acceptance & Binding Agreement

By checking the acceptance box and completing enrollment, I acknowledge that:

  • I have read and understood this Agreement

  • I agree to be legally bound by its terms

  • My acceptance is effective upon submission of payment

This Agreement constitutes the entire understanding between myself and Longevity Docs LLC regarding participation in the CLD Program.
